Accessing Your Medical Record
If you are a patient of Warner Hospital & Health Services, you can access your medical record through the patient portal, MyHealth. This online tool gives you access to your medical record/information from virtually anywhere at any time. Enrolling in the patient portal, MyHealth allows you access to the following:
- Review Lab and Radiology Results
- View Medications
- Request Updates to Personal Information
- See Visit History and Discharge Information
- View, Print, or Transmit Medical Information
- View Billing Information
MyHealth is completely secure, so you can be confident that private information is protected.
How to get signed up?
You can ask for a sign-up packet in Patient Registration or Health Information Management. For your convenience you can also complete the Patient Portal Enrollment Request found at the bottom of this page and return to email@example.com.
After you are registered for this access you will be sent a temporary username and password. You must access the portal within 72 hours, or your username will expire. You will create your own username and password which is known only by you.
Access to MyHealth allows you to partner with your physician and access the most up to date patient information in real time. Any questions about MyHealth, please contact the MyHealth helpline at 217-937-5206.
Health Information Management complies with the HIPAA Privacy standards and will not release your private health information without the proper authorization. You may obtain copies of your medical records by contacting Health Information Management at 217-935-9571 x 3305. The Warner Hospital & Health Services and Family Medicine Release of Information Authorizations are available for download. You may complete the authorization and fax to 217-935-4928.
Health Information Management hours are Monday–Friday from 7:00 a.m. to 4:00 p.m.
